【臨床研究】
(1) 膝関節
末期変形性膝関節症に対し、人工膝関節全置換術あるいは単顆型人工膝関節置換術を行っています。良好な術後成績を得るためには、正確な骨切り・コンポーネントの設置および適切な軟部組織バランスの獲得が重要とされています。正確な骨切り・コンポーネントの設置に関しては、Navigation systemを導入・3次元運動を容易に計測できるように独自に改良し、また3次元術前計画ソフトウェアを導入し、その正確性を向上させています。適切な軟部組織バランスの獲得に関しては、独自のtensorを開発し、術中の軟部バランスを評価・調整しています。このtensorは大腿骨コンポーネントを装着し、膝伸展機構を修復し、術後膝を再現した状態で軟部組織バランスを評価できるtensorであり、国内外で高い評価を受けています。現在ではこのtensor systemとnavigation systemを同時に併用することで、正確な骨切り・コンポーネントの設置および適切な軟部組織バランスの獲得が同時に得られるようになり、さらに3次元運動と術後成績、また術中のパラメータとの関係と評価することが可能となっております。これまで、我々は人工膝関節全置換術における手術中の靭帯バランスが術後の靭帯バランスや関節可動域と関係すること、Navigation systemの臨床成績、術後の満足度、術後感染の原因や治療成績などを報告してまいりました。単顆型人工膝関節置換術においても独自に開発したtensorにて靭帯バランスを評価し、術後の臨床成績との関係を研究しております。また、日本人の下肢アライメントの大規模調査を行い、年齢による変化や変形性膝関節症による変化など日本人の特徴を報告しております。これらの研究成果をふまえて、人工膝関節置換術を受けられる患者さまの術後成績の向上、満足度のさらなる向上を目指しています。
(2) 股関節
変形性股関節症に対する人工股関節全置換術は優れた除痛効果など安定した治療成績が期待されます。またインプラントデザインや慴動面の改良、材質の向上など長期成績についても良好な結果が期待されております。しかしながら、インプラントデザインによる固着部位の相違によりインプラント周囲の骨密度が低下する問題点が指摘されております。そこで人工股関節全置換術後から大腿骨を中心とした骨密度を測定し、より生理的な荷重伝達を可能とするインプラントの選択等、治療成績向上に向けて研究を行っています。

寛骨臼形成不全に対する骨切り手術(寛骨臼回転骨切り術)では、Cerved Periacetabular Osteotomy (CPO) を採用して治療に当たっております。この術式では皮切が9cmであり股関節外転筋力への影響が極めて低いことから低侵襲手術であると考えられる一方、骨切り線や寛骨臼の回転方向・量を術者の経験などに頼ることが問題点でした。そこで現在、3Dシュミレーションソフトならびにナビゲーションを併用して手術を行っております。適切な骨切り線や荷重環境の獲得、さらに手術成績の標準化を目指しています。
【基礎研究】
(1) Biomechanics
人工膝関節においては、Navigation system、Tensor systemを併用した手術、2D-3D image matching techniqueを用いて三次元解析ソフトを用いて人工膝関節の3次元動作解析を行い、同時に術前パラメータ・術中操作・術中パラメ-タ・術後成績の関連を解析し、より生理的に動く、そしてより曲がる膝を目指して研究しています。また、東京女子医科大学 先端生命医科学研究所(http://www.twmu.ac.jp/ABMES/FATS/)と共同研究を行っており、軟部組織バランス測定ロボットテンサーを開発中です。
(2) Biology 変形性股関節症における軟骨損傷の病態解明
変形性関節症に対する手術的治療として人工関節置換術は非常に良好な成績を収め、確立されたものとなっていますが、有効な保存的治療(内服、遺伝子治療等)は確立されておりません。よって我々は軟骨変性に対し、病態の解明ならびに薬物ならびに遺伝子治療を開発することを研究の目的としております。 変形性関節症の発症にはさまざまな遺伝子が関与しており、我々はその中で癌抑制遺伝子であるp53さらにp53関連分子のp38 MAPK, p53R2 が関与するという非常に興味深い新しい知見を得ました。その後p53に関連してp21, PTENなどの分子に着目し変形性関節症の病態解明および進行予防を含めた軟骨治療戦略への応用につなげていきたいと思っております。

これらの研究成果を日本整形外科学会、日本整形外科基礎学術集会、日本人工関節学会、日本関節鏡・膝・スポーツ整形外科学会(JOSKAS)、日本股関節学会、日本臨床バイオメカニクス学会、日本CAOS (Computer Assisted Orthopaedic Surgery)研究会、日本軟骨代謝学会、アメリカ整形外科学会(AAOS)、国際人工関節技術学会(ISTA)、International Congress for Joint Reconstruction (ICJR)、Computer Assisted Orthopaedic Surgery (CAOS) International、国際関節鏡・膝・スポーツ整形外科学会(ISAKOS)、アメリカ整形外科基礎学会(ORS)等で報告し、新たな人工関節の開発を目指しています。
