健康診断は予約不要です。 ※朝、昼前後が比較的混雑する傾向にあります。混雑している場合に備え、時間に余裕をもってお越しください。 It tends to be relatively crowded in the morning and around noon. ４月６日(月)１３時 ～ ４月１０日(金)１７時まで 下記①～⑥の提出書類確認後に学生証等を配付します。必ず上記期間内に受取にきてください、 From Monday, April 6, 1:00pm to Friday, April 10, 5:00pm Student ID cards and other materials will be distributed after confirming the submission of documents ① through ⑥ listed below. Please make sure to come and collect them within the specified period |

| 次の①～⑥の書類を記入して持参してください。*②は個人情報提供を希望されない方のみ 書類の提出がないと，学生証，アカウント通知書，健康診断票，採尿容器， オリエンテーション資料をお渡しできません。 ・②の書類は送付した「入学の手引き」にも綴じられています。 ・③～⑥の書類はダウンロードし提出してください。 If the documents are not submitted, we will not be able to provide the student ID card, account notification, health examination form, urine sample container, and orientation materials. ② form are from Guidebook for enrollment. ③to ⑥ form are please download and submit them to the Graduate School Academic Affairs and Student Affairs Section. |

| 通学定期（居住地の最寄駅・バス停から神戸大学医学部キャンパスの最寄り駅・バス停まで購入可能）の購入を希望される方は，学生証等受取時に申し出てください。 「通学証明書交付願」をお渡しします（学生登録票の提出必須）。 ※通学定期は通勤やアルバイト等のために購入することはできないので注意してください。 If you wish to purchase a commuter pass (from the nearest station or bus stop to your residence to the nearest station or bus stop for Kobe University Medical School Campus), please inform us when receiving your student ID card and other materials. We will provide you with a "Request for Issuance of a Commuter Certificate" (submission of the Student Registration Form is required). *Please note that commuter passes cannot be purchased for purposes such as commuting to work or part-time jobs. |
