| 準備物 | ①学生証 ②アカウント通知書（白い封筒 学籍番号が記載されているもの） ③修学案内 ④大学院医学研究科オンライン授業受講マニュアル ⑤学生生活案内（医学部学務課作成） |
| 注意事項 | ログイン後は自分の音声をミュートにして待機してください。 |

４月７日(月)１３時 ～ ４月１１日(金)１７時まで 下記①～⑦の提出書類確認後に学生証等を配付します。必ず上記期間内に受取にきてください、 From Monday, April 7, 1:00pm to Friday, April 11, 5:00pm Student ID cards and other materials will be distributed after confirming the submission of documents ① through ⑦ listed below. Please make sure to come and collect them within the specified period |

| 次の①～⑦の書類を記入して持参してください。*③は個人情報提供を希望されない方のみ 書類の提出がないと，学生証，アカウント通知書，健康診断票，採尿容器， オリエンテーション資料をお渡しできません。 ・①～③の書類は送付した「入学の手引き」に綴じられています。 ・④～⑦の書類はダウンロードし提出してください。 If the documents are not submitted, we will not be able to provide the student ID card, account notification, health examination form, urine sample container, and orientation materials. ①to ③ form are from Guidebook for enrollment. ④to ⑦ form are please download and submit them to the Graduate School Academic Affairs and Student Affairs Section. |

| 通学定期（居住地の最寄駅・バス停から神戸大学医学部キャンパスの最寄り駅・バス停まで購入可能）の購入を希望される方は，学生証等受取時に申し出てください。 「通学証明書交付願」をお渡しします（学生登録票の提出必須）。 ※通学定期は通勤やアルバイト等のために購入することはできないので注意してください。 If you wish to purchase a commuter pass (from the nearest station or bus stop to your residence to the nearest station or bus stop for Kobe University Medical School Campus), please inform us when receiving your student ID card and other materials. We will provide you with a "Request for Issuance of a Commuter Certificate" (submission of the Student Registration Form is required). *Please note that commuter passes cannot be purchased for purposes such as commuting to work or part-time jobs. |

※持ち物「３．健康調査の用紙」はダウンロードして両面印刷して持参してください。 ＊Please DL the 3.Health Survey Form, print it double sided, and bring it with you. 健康診断を受診するためには事前予約が必要です。 次のURLにて，４月22日 12：00 より予約受付を開始します。 An advance reservation is required to undergo a health checkup.
